Referring Patients or Telephone Consultations
Our Center and Doctors of Optometry are available as a resource for other professionals in the community, either for referral or telephone consultation.
General Procedures
When referring patients to our Center for special services/testing, please provide us with all pertinent patient data and ocular findings. Be as specific as possible with regards to the services and special testing requested. Fax any paperwork or reports to 323.235.6203 or mail this information to us in sufficient time to arrive prior to the patient's appointment. The patient will have to be rescheduled and services delayed if the needed information is not received prior to the patient visit. Upon completion of the requested services, we will promptly refer the patient back to the referring doctor for the continuation of care. A written summary report of our findings will be sent to the referring doctor.
Glaucoma Collaboration
Forward the Treatment Plan and associated forms (request for special testing, etc.) to OCLA. Upon completion of initial consultation, the patient will be returned to you for continuation of the management of his/her glaucoma condition.
Telephone Consultation
Telephone consultation is available with one of our experts regarding a particularly challenging patient. We have an excellent faculty, exceptional credentials and expertise in all areas of optometric eye and vision care.
