What should I bring to my appointment?

  • Bring all prescription glasses, contact lens and sunwear.
  • Bring a list of all prescribed and over the counter medications currently used.
  • Bring all vision and medical insurance cards along with your picture I.D.
  • Bring all referral forms from your medical doctor.
  • Bring a list of questions and concerns.
  • Bring co-payments, deductibles and payment for non covered services as these are required at the time services are provided.
